The accessible information standard requires all NHS service providers by law, to record any specific communication needs to enable all patients to receive information in a suitable format. To provide support and assist with any communication needs. Record this information and share with other NHS and adult social care providers with their consent.
The Standard aims to ensure that all people with a disability, impairment or sensory loss have access to information they can understand and the communication support they may need, this includes parents and carers of patients who have such communication needs.
Lime Tree Surgery and Lime Tree Surgery Durrington; ask all new patients who register at our practices to tell us about their specific needs on our registration form.
All staff use online training to help us improve the service we offer and to gain a greater understanding about the Accessible Information Standard and how we implement and act on this for our patients.
How we can help; arrange for an interpreter to attend appointments, email, call or send text messages, print information or letters in a larger print, we also offer online appointment booking and repeat prescription ordering, online access to summary medical record.
We will ensure any communication requirements made by our patients are clearly noted on medical records and acted on wherever possible and shared with other medical services, this is to help ensure other medical services are aware of how to communicate or offer information to our patients in the most suitable way for them.
If you would like to discuss your individual needs or are unsure of how we can help you please contact us, or inform us of your current information and/or communication needs.
