Sick Note
First 7 days: No need to see your GP, just complete a
self certificate form (obtained from your employer or
GP). A private sick note can be provided by the GP but a
charge will be made.
Longer than 7 days: You will need to see the GP for a
certificate. Please book an appointment.
Please note that if you require time off work following
hospital treatment, the hospital should provide you with
a certificate – please remember to tell the hospital if
you need a certificate.
