How to Register Your Household:
We will register anyone living within our practice
boundary. You may be
asked to provide evidence of residency. Please bring your
medical card to the surgery; if this has been mislaid we
will still be able to help you. As no one is
automatically registered with a surgery, it is important
that both you and all members of your household register
at the same time. We recognise the important role of
carers in looking after dependent relatives; please make
it known if you are caring for someone.
Who will be your Doctor?
Your registration will be with the practice and you can
see any of the doctors practising at the surgery,
although you may have to wait longer to see a specific
doctor. For continuity of care, we would encourage you
to consult the same doctor as far as possible.
Your First Appointment
When you register with the practice you will be asked to
complete a health questionnaire (one for each member of
your household). If you wish, you can make an
appointment for a new patient check.
The
Practice Area
The practice welcomes new patients
living within our boundary. The practice boundary line
extends as far west as Titnore Lane, to the South as far
as the railway line and east to Sompting Road. All areas
north of this boundary, including Patching/Clapham are
within the practice area. Patients moving out of the
practice area will routinely be asked to register at a
nearer practice.
